A valid government-issued photo ID (driver's license, passport, or state ID), the unsigned document(s) you need notarized, and any other signers if the document requires them. Don't sign the document beforehand — it must be signed in front of the notary.

Yes. We regularly notarize at hospitals, nursing homes, hospice care, and assisted living facilities. The signer must be conscious, of sound mind, and able to communicate willingness to sign.
Many documents (especially wills and certain affidavits) require additional witnesses. Witnesses cannot be the notary or anyone named in the document. Let us know in advance and we'll guide you on what's needed.
